Welcome to the Survey of Consumer Experience of Home Services

The Aged Care Quality and Safety Commission (Commission) is planning to visit the provider who is responsible for delivering aged care services to you. The purpose of the visit is to assess whether the quality of care and services being provided to you and other consumers meets the Aged Care Quality Standards.

The Commission is a separate entity to MyAgedCare. The Commission does not determine your level of care, funding or government benefits, and none of these will be affected by any feedback you choose to give. As part of this visit, you would have received a letter inviting you to complete this survey.

This survey seeks to understand how your experience of the home services i.e. your Home Care Package or Commonwealth Home Support Program service.

Please note the Commission ID (or RACS ID) that was provided to you in the letter or poster. You will need to have this ID number on hand before you complete the complete the survey, as it enables us to identify your service.

You may choose to remain anonymous when you complete this survey.

The following kinds of personal and sensitive information may be collected (optional):
• your name, address, gender, and contact or identity details
• your feedback about your aged care service

The responses you provide will be included in the information used in a site visit to assess your aged care service’s performance under the Aged Care Quality Standards. Where you have indicated that you consent for follow up of your survey, you may or may not be contacted by a member of the team conducting the Quality Audit of your service.

We will not use or disclose your personal information for a purpose beyond this survey unless we obtain your consent or the use or disclosure is permitted under the Privacy Act 1988.

The feedback you provide will be treated as strictly confidential and in accordance with the Commission’s Privacy Policy.
