This survey is intended for the person responsible for Occupational Health and Safety (OHS) in your workplace, specifically around Ultraviolet Radiation (UV) and sun protection.

Participation is anonymous, unless you wish to share your contact details with us. This survey is confidential - no workplaces or employers will be named in any reports.
